Community Partnership Form

Please complete this form when you engage a community partner program or service in your school—before, during or after school hours and would recommend it for other Lights ON Community Learning Centers.  The information will be posted on the Lights ON website under the Community Resources 

Media Request Form

Please fill out this form when you have a special event at your school and would like to have media coverage.  Please email Mary McNeil and at m.mcneil@mchsi.com or fax to Mary at 309-277-0069 who will write and submit a press release. 

Program Update Report Form

Please fill out and bring to the bi-monthly Lights ON Site Coordinators’ meetings or e-mail the information to Maxine Russman at mrussman@riroe.k12.il.us.  This information will be included in the composite Lights ON Program Report that will be posted on the Lights ON website and viewed by community stakeholders.
